The Community Learning Disabilities Team operates an open referral system, accepting referrals from General Practitioners, relatives, social workers as well as from individuals themselves.

The CLDT provides services mainly for adults. Other professional groups provide specific paediatric services. For further information about this you can contact Mary Paris, Business Manager, Child Health Service on 01383 565320. Please also refer to our Transition Nurse section under the Community Team for Learning Disabilities information on our pages which provides further information on support for transition from child to adult learning disability services.

The Community Team for Learning Disabilities (CTLD) will only accept referrals where it is clear that the person has a learning disability, or where a specialist assessment is necessary. The CTLD does not offer services to people with more specific learning difficulties or developmental disorders such as Attention Deficit Disorder, Aspergers syndrome, Dyslexia, or Tourettes syndrome, although sometimes people with a learning disability also have these conditions.

Referrers to the CTLD should ensure that they include information in their referral which provides evidence that the person has a learning disabliity, or should make it clear that an assessment is required to determine this. Sometimes it is obvious that the person has a learning disability, or they may have carried out assessments during childhood, and at other times it is less clear. Sometimes a formal assessment can make it clearer whether the person has a general learning disability and needs the services of the CTLD. The formal assessment can include paper and pencil tests with a psychologist, and a questionnaire completed by a relative or carer, as well as a chat about when the disability started.

The Community Learning Disabilities Team operates a single point of access. If you would like to make a referral, please download and complete the referral form which is available on the link below and email or post to our Referral Co-ordinator:
