District nurses play a key role in supporting people in their homes, meeting a wide range of health care needs.

District Nurses assess, plan, implement and evaluate nursing care with patients, their family and carers.Patients include:

  • patients discharged from hospital
  • terminally ill patients
  • chronic sick and disabled
  • patient with diabetes, hypertension and incontinence carers

District Nurses work as part of a multidisiplinary team, which include GP's, Health Vistors, Homecarers, MacMillan Nurses, Specialist Nurses, CPN's etc.
