The main support you can expect from both teams within the Community Cardiac Specialist Nurse Service is outlined on the Community cardiac specialist nurse service page.

The specialist nurse team for heart failure will;

  • Teach you to monitor heart failure symptoms and recognise signs that your body may be retaining additional fluid.
  • Support you to manage symptoms & medications.
  • Work closely with your Heart Failure Cardiologist and your GP to provide an individualised treatment plan.
  • Work to reduce unnecessary hospital admissions & readmissions.
  • Offer support to patients with palliative care & end of life needs when appropriate.
  • Adjust treatment plans to meet individual needs.

Referrals

Referrals can be made by your GP practice, hospital or other health care professionals. Patients with heart failure who have previously been seen by the team can self refer at any time.

Location information 

Community Cardiac Specialist Nurse Service work across two sites. We review patients by telephone, in clinics or if required at home.
