Patient Education for people living with Type 1 diabetes in Fife 

This information is provided through the hospital diabetes service.

Patient Education for people with type 2 Diabetes

For patients with newly diagnosed Type 2 (Control It)

Criteria for referral: 

•    All patients newly diagnosed with Type 2 diabetes
•    Patients with existing Type 2 diabetes who would benefit from a short programme of structured patient education

Course Content

This interactive session is delivered by dietitians with expert knowledge of diabetes and skills in delivering education. They are held in various venues across Fife each month.  There is an opportunity for discussions around topics that are most relevant to patients. The group setting allows patients to share challenges and successes and build confidence, reinforcing that they are not alone.

Patients will come away from the experience with plans to enable them to manage their condition better.   

Topics covered include:

  • What is diabetes?
  • Medications and health assessments
  • Eating in a healthy and balanced way
  • Tips for managing lifestyle including keeping active

You can book a place on Control IT by filling out the self referral form  or contact us by email: fife.diabeteseducation@nhs.scot 
or telephone: (01592) 226736.

You can also ask your GP or nurse to refer you.

Ongoing Education for patients with Type 2 diabetes (Diabetes explained)

Diabetes Explained is a more comprehensive Structured Patient Education Programme delivered over 4 interactive half day sessions delivered by the Diabetes Specialist Dietitians.  Participants have opportunities to set and review goals for self-management around the weekly topics.

Criteria for Referral:

·        Patients with established Type 2 diabetes who would like further education and support to manage their condition

Topics covered include:

  • What is diabetes
  • Digestion and blood glucose
  •   Health results and explanations
  • Medications for diabetes
  • Weight management
  • Healthy eating and exercise
  • Carbohydrate awareness and food labels
  • Hypoglycaemia and hyperglycaemia
  • Preventing diabetes complications
  • Driving/travel/sick day rules
  • Looking after my diabetes, care planning and goal setting

This programme has been shown to improve blood glucose control; cholesterol levels; quality of life; dietary intake and physical activity.

Information and education for people living with or affected by diabetes.

My Diabetes, my way

You can access free online education courses to learn more about diabetes by visiting the My Diabetes My Way eLearning Website. These courses will be especially useful if you've just been diagnosed, or want to know more about any of these topics:

  • Type 1 diabetes
  • Type 2 diabetes
  • Gestational diabetes
  • If you are considering, or are starting on an insulin pump
  • If you are considering, or starting FreeStyle Libre
  • If you are a teenager/young adult with Type 1 diabetes

Diabetes UK Learning Zone

Active options - health and wellbeing

Active options - Heath and wellbeing

Active Options is a physical activity programme for people in Fife who have a long term health condition. The aim of the programme is to get you moving a bit more. Classes include:

  • Active for Health
  • Active Circuit
  • Active Strength and Balance
  • Active Move
  • Active Back Care
  • Active Stability

The Health and Wellbeing programme is self-funded. Classes are run by highly-qualified instructors. Fife Sports and Leisure Trust deliver over 90 health classes a week, with 3000 participants attending and enjoying health and wellbeing benefits of being active, as well as meeting others who are also living with a long-term condition. To be referred into the Active Options programme, please ask your health or social care professional or make a self referral.

Depending on what type of diabetes you have, you may already be using technology to help you monitor and manage your diabetes. NHS Fife offers several options.
