Why identify frailty
Frailty is not a static state and can change in an individual person, and can get better or worse. Someone living with frailty may need to adapt how they live their life, and find new ways to manage day to day tasks. This can be true for their family too.
Screening for frailty
Identifying frailty can result in early detection which can help you make changes, plan or put support in place that can:
- Improve outcomes of individuals
- Support an individual to live well at home
- Avoid unnecessary harm
- Potentially reverse the severity of frailty
Clinical Frailty Scale
The Clinical Frailty Scale (CFS) is a scale that measures frailty based on function.
The scale has nine points from 1: Very Fit to 9: Terminally ill.
