Community respiratory service

The Community Respiratory Team (CRT) covers the whole of Fife. The team comprises of Nurses, Physiotherapists, Health Care Support Worker, and supports people with a confirmed diagnosis of COPD, ILD and Bronchiectasis.  


Referral routes

We accept referrals from health care professionals, including, GP, ANP’s, Practice Nurse, DN’s, AHP’s, SAS. People must be registered with a Fife GP practice.  

Location

The teams working hours are 9am-4.30pm Monday to Friday, excluding public holidays. 

Community Respiratory Team

The team has 3 main work streams: 

  • Prevention of admission and acute exacerbation support for patients in their own home 
  • Supported discharge from hospital 
  • Chronic disease management
