Who is the Care Home Liaison Nurse Team? 

We are a team of experienced Registered Nurses and Health Care Support Workers who work collaboratively alongside care home staff, supporting them to ensure each resident receives safe, effective and person centred care.

Who we are not 

Although we work closely with our mental health colleagues, we are a separate service from the Mental Health Care Home Liaison Nurse Team.

What do we do? 

The team aims to provide a visible and accessible service which is responsive to the needs identified by staff in individual care homes in areas such as facilitation of training and education. We can provide educational sessions on various topics central to delivery of care for care home staff, including: 

  • Pressure ulcer prevention 
  • Nutrition
  • Medication management
  • Falls prevention
  • Bowel management
  • Continence and catheter care
  • Restore 2
  • GERT (Step Into Their Shoes) frailty simulation
  • Dysphagia
  • Autonomy and accountability
  • Wound management 

We also aim to act as a resource for care homes and offer links between care home providers and the wider health and social care services that support the residents of Fife. We can be the first point of contact for advice and can help prompt support when required.
