Frailty is a distinctive health state related to the ageing process in which multiple body systems gradually lose their in-built reserves.

People living with frailty do not recover from an illness, accident or other stressful event as quickly as they used to, putting them at risk of adverse outcomes (British Geriatrics Society / Royal College of General Practitioners).

The Frailty Managed Clinical and Care Network (MCCN) is focussed on prevention, early intervention and integrated support and care at a local level. The MCCN is building on existing collaborative work.
